The Player is responsible for completing the form and agreeing to the liability waiver that will appear at the bottom of the form.
INSTRUCTIONS
- Select the Division first, then the Team you are registering to, then click Submit.
- Answer the question about whether you played in a BCSA-registered fall/winter team. The rest of the form will then appear.
- You must fill out all parts of the form as accurately as possible. Incomplete forms will not be accepted.
- Please enter your name as it appears on your driver's licence or passport.
- Make sure your email address is correct . You will receive an email confirmation of your registration.
- If you don't have an email please use your manager's email address to receive your confirmation.
