Forms

These forms are all in either Adobe pdf or Microsoft Excel format. To view and print them you will need to have the Adobe Reader version 5.0 or later installed on your computer and/or a copy of Microsoft Excel (or OpenOffice). Adobe Reader is available for download free from www.adobe.com

Player Registration

All players must sign a “C” form to be eligible for summer play.
Forms have to be given to a League official 24 hours before play.
Players can play for two teams with the approval of JB.
Registration and transfer deadline is Monday, June 5, 2006, 10:00 PM.
Players not registered in BCSA winter play pay $12 each.. 
The summer team registration with BCSA is paid from your league fees.
Youth players must submit signed Parental Liability form before play.
Players must have ID (Government or Senior League) at each game.
Teams playing un-registered/suspended players will be fined and lose points.
Players suspended by BCSA are ineligible to register or play.
Youth players still in Provincial Cup  must be signed on permit for each game.

Under 21

Players must be born after Dec. 31, 1985
Three overage Knight League players are allowed to play per game.
